Whether you are managing a long-term physical condition, navigating a mental health challenge, or dealing with both at the same time (as is very often the case), our GPs provide structured, compassionate, and thoroughly coordinated care. We do not treat conditions in isolation — we treat people.

Mental Health Care at Haan Health

GP Mental Health Treatment Plan (GP-MHTP) A GP Mental Health Treatment Plan is a formal plan prepared by your GP to support your mental health care. It allows eligible patients to access up to 10 individual Medicare-rebated sessions with a registered psychologist or other mental health professional each calendar year (subject to clinical review after initial sessions). Our GPs take the time to prepare these plans carefully, with a thorough assessment of your situation and clear, collaborative goal-setting.

Assessment & Diagnosis Many patients come to us not yet sure what is happening — just feeling that something is not right. Our GPs are trained to assess depression, anxiety disorders (including generalised anxiety, panic disorder, and social anxiety), PTSD, OCD, and other mental health conditions, and to work with you on the right path forward.

Medication Management For patients where medication is appropriate as part of their mental health treatment, our GPs prescribe and review mental health medications carefully, with regular monitoring and follow-up to ensure the approach remains right for you over time.

Mental Health for Specific Groups We provide targeted mental health support for groups with specific needs, including perinatal mental health (during and after pregnancy), men’s mental health, adolescent mental health, and LGBTQIAP+ mental health — with a particular focus on creating a safe, affirming space for every patient.

Chronic Disease Management at Haan Health

GP Management Plan (GPMP) & Team Care Arrangement (TCA) A GP Management Plan is a structured document that outlines the goals, treatments, and services involved in managing a patient’s chronic condition. A Team Care Arrangement coordinates the involvement of at least three health professionals (including your GP) in your care. Together, these allow eligible patients to access Medicare-rebated allied health services — including physiotherapy, dietetics, and psychology — as part of coordinated chronic disease management.

Diabetes (Type 1 & Type 2) Diabetes requires consistent, proactive management to prevent complications. Our GPs provide structured diabetes care including HbA1c monitoring, medication review, foot care assessment, referrals to ophthalmologists and endocrinologists, and coordinated support via Team Care Arrangements.

Hypertension (High Blood Pressure) High blood pressure is often symptomless but carries significant cardiovascular risk. Our GPs manage hypertension through regular monitoring, lifestyle guidance, and medication management as needed — with the goal of sustained, long-term blood pressure control.

Cardiovascular Disease For patients with established heart disease, our GPs provide ongoing monitoring, medication management, and coordination with cardiologists and other specialists, as well as structured care plans aligned with national clinical guidelines.

Asthma & COPD Effective management of asthma and COPD requires regular review, correct inhaler technique, action planning, and spirometry monitoring. Our GPs provide structured respiratory care and coordination with respiratory specialists where needed.

Hyperlipidaemia (High Cholesterol) Cholesterol management is a key component of cardiovascular risk reduction. Our GPs assess lipid profiles, advise on dietary and lifestyle modifications, and prescribe and monitor lipid-lowering therapies as appropriate.

Osteoporosis & Bone Health Particularly important for post-menopausal women and older adults, bone health assessment includes bone density monitoring, calcium and Vitamin D assessment, and fracture risk reduction strategies.

How many psychology sessions can I access with a Mental Health Plan?
